‘Errors and misjudgements’ in mental health care led to discharge of man who killed three, as ‘risk to public not managed well’

A “series of errors and misjudgments” in mental health care led a man being discharged, despite repeatedly not taking medication and showing signs of aggression, months before he killed three people in Nottingham, a report has concluded.

A review by the Care Quality Commission (CQC) of the treatment that Valdo Calocane received from Nottinghamshire Healthcare NHS Foundation Trust found that “the risk he presented to the public was not managed well”.

He killed Barnaby Webber and Grace O’Malley-Kumar, both 19, and 65-year-old Ian Coates, in Nottingham on 13 June, 2023.

Today, the families of the victims, who are represented by Hudgell Solicitors, said in a statement that those responsible for failings in Calocane’s care have “blood on their hands”.
